US Hospitals. Vicksburg Miss.
Sir: I would respectfully request you to examine the enclosed certificate as to disability of Ebenezer Bradley, private in Co B. 124th Regt Ills. Vols. (now absent on furlough) and if possible cause him to be discharged from service. Nothing but absolute disability would impel him or his friends to apply for his discharge. I will feel thankful for your special notice of this case and if any further certificates are necessary please address Hon. Saml. D. Lockwood, Batavia Illinois instructing him how they should be made &c
