I Robert A Henderson Co. C. 36th Ills. Vol. Inft was crippled while engaged in takeing of the railrode South of Atlanta in left rist and breast which Renders me unfit for activ service. and I would like to be transferred to quincy Ills as that the nearest hospital to my residence. you will oblidge your obediend servant by answering those few lines.

I came to this hospital on the 20th of October

And have been standing guard ronnd the hospital sints the first of December with one arm in a sling
