Dear Sir

This is to certify that James A Seaton a member of Company 78th reg Ill Vol. has called me to treat him professionally. He is afflicted with hemorhoidal tumors, bad cough etc etc which in my opinion renders him entirely unfit for further Military duty. He desires a discharge from the service which I think ought to be granted him
